In order to make sure your visits and insurance billing go smoothly, below is some important information about the administrative side of the practice.
The location is steps from Grand Central at 260 Madison Avenue (between 39th and 38th Streets) on the 8th floor.

Appointments are scheduled with Dr. Michelle Perepiczka by phone or email. Hours of availability are the following:
Monday 9am to 5pm
Tuesday 9am to 5pm
Wednesday 9am to 5pm
Thursday 9am to 5pm
Friday 9am to 5pm
Mental Health Counseling Services of New York accepts private pay clients on a sliding scale. Contact Dr. Michelle Perepiczka for sliding scale information.
Payment for sessions, co-pays, and deductibles are accepted in cash and check.

What to Bring To Sessions
Remember your insurance information.
In order to begin receiving services promptly, it is requested that you bring their current insurance information with you for the initial visit. If you have any changes in insurance along the way, please bring in the new information.
Remember to bring copies of custody or divorce papers (if applicable) if you are bringing in a child for counseling.
Copies of these forms can help ensure privacy of children and protect parental or caregiver rights to information about children. Please bring current paperwork with you for the initial visit and copies of any new information down the road.
